MABEL Questionnaires
Below are the main survey materials. There are 16 versions of the questionnaire. Four versions are for each type of doctor, and the discrete choice experiment was split across another four versions for each doctor type. So for the Specialist survey, there are 4 versions and these differ only with respect to the discrete choice experiment questions in Section B. All other questions are the same.
