I agree to complete the MABEL survey. I have read the Plain Language Statement which was mailed to me, and which I keep for my records.

• agreeing to take part means that I am willing to complete an online survey or paper survey asking about my work hours, work choices and job satisfaction
• my participation is voluntary, that I can choose not to participate in part or all of the project, and that I can withdraw at any stage of the project without being penalised or disadvantaged in any way
• this is a longitudinal survey and I will be asked to participate again next year, although I am under no obligation to do so
• any data that the researcher extracts from the survey for use in reports or published findings will not, under any circumstances, contain names or identifying characteristics
• any information I provide is confidential, and that no information that could lead to the identification of any individual will be disclosed in any reports on the project, or to any other party
• data from the survey will be kept in a secure storage and will be accessible to the research team only.
• data from the survey may be made available in de-identified format for use in other research projects by other researchers on a range of medical workforce issues, with ethics approval.
